Service Introduction
Springboard Project Transitional Support Service for Persons in Mental Recovery (SP-TSS) provides multi-disciplinary and transitional support for service users who are waitlisted for places in halfway house services. Through proactive outreach, continuous motivational enhancement, individualized training plans and peer support, the service integrates a recovery oriented approach with close medical-social collaboration to facilitate their smooth transition into halfway house services and other community support services.

Our Aims
We are dedicated to delivering "Care for People” and "Recovery-oriented” services that enable people in recovery by:
● Provide seamless and timely transitional support to service users awaiting admission to a halfway house, facilitating their smooth transition into the halfway house service through a multidisciplinary outreach team with a medical social collaboration approach;
● Provide structured, multidisciplinary interventions to promote the recovery of service users according to their specific needs and circumstances while awaiting admission to a halfway house.

● Orientation programmes and transitional support prior to halfway house admission
● Ongoing support after admission to halfway house
● Connecting service users who leave halfway house services within 6 months with community support services to facilitate their smooth adjustment to community living
● Therapeutic and support groups and activities
● Family/ caregiver support
● Life skills training
● Peer support
● Referral to other services
Service boundary
● New Territories including Shatin District, Tai Po District, North District, Yuen Long District and Tuen Mun District
Service Target
● Persons who are wait-listed for placement in the HWH Services under the CRSRehab system of the SWD
● Service users whose residential addresses are within the service area
Service Charge
● The core services specified under the Funding Service Agreement are free of charge;
● Social and recreational activities will be charged on need basis. Fees and payment procedures will be notified to service users in advance through notices, posters and electronic platforms.
Service Application
● Service targets and/or their family members/ carers can approach our service directly via phone or in person;
● Through referrals by medical practitioners, social workers, and other paramedical workers (Download referral form)
Exit of Service
● The service user achieves the service objectives (e.g. living in Halfway House for 6 months, or settled in community or having bridged to appropriate community service); or
● The service user requests termination of service from their responsible social worker; or
● The service user loses contact or is unable to receive service
Service Hours
● Monday to Wednesday, Friday to Saturday 10:00 a.m. to 6:00 p.m.
● Thursday - 2:00 p.m. to 10:00 p.m.
● Services will be provided on Sundays and public holidays as needed
