Life Plan for Persons with Special Needs under Special Needs Trust

Service Introduction


As an institutional carer under the Special Needs Trust Scheme of the Social Welfare Department, New Life Psychiatric Rehabilitation Association (New Life) will properly implement the Life Plan formulated by the Settlor, so that Persons with Special Needs can receive long-term life care and professional support. We will also provide regular assessments to monitor the conditions of the Persons with Special Needs and arrange optimal services.

People in recovery of mental illness or people with other disabilities (including autism spectrum disorders, ASD service users), aged 15 or above

Scope of Service

Case Management Service

  • Case Manager, an experienced registered social worker, will implement the formulated Life Plan
  • Provide regular assessments for the service user, understanding his/her needs in daily living, education, employment, training and medical aspects, etc.
  • Arrange follow-up interview through phone or face-to-face visit

Additional Services

Home Care Service
Daily Living, Learning, Medication, Social and Entertainment Needs

Professional Service
Service Referral, including Clinical Psychology Services, Occupational Therapy Services, Speech and Hearing Services or other professional services

Others
  • Arrange instructors for professional training, for instance, financial arrangement, employment support, social competence, emotional management and other training
  • Arrange individual coach, travel and other activities
