 A Brief Introduction to the Service

Help service users sustain independent living and reintegrate into society through outreaching visits, training and programmes
 
Enhance the working ability, interpersonal and social communication skills of people with mental illness
 

Community Mental Health Care Services, commenced in October 2005, was subvented by the Social Welfare Department. Shan King Halfway House and Chuk Yuen Halfway House provide services for people with mental illness who reside in the areas West of the New Territories and Kowloon West respectively. We help people with mental illness discharged from psychiatric wards/hospitals and halfway houses to reintegrate into the community through outreaching visits, counseling, groups and programmes. In addition, we also arrange day programmes and provide tailor-made vocational rehabilitation services for idling Halfway House residents awaiting placement in vocational rehabilitation services, so as to develop and maintain their social and vocational functioning skills. To achieve optimum rehabilitation results, we conduct needs assessment and develop individual rehabilitation plan.

  Aged 15 or above
  Male/Female
  People with mental illness who have been discharged from psychiatric wards/hospitals for less than two years or discharged from halfway houses
  People with mental illness newly admitted to halfway houses without a day placement or idling halfway house residents awaiting vocational rehabilitation services
  Referral can be made by psychiatrists, social workers or allied health workers
  Provide training in social and communication skills, budgeting, home management skills and education on mental health through outreach visits
  Provide occupational therapist-led vocational training to develop their work habit and conduct regular assessments and follow-up services
  Help people with mental illness to adjust to new environment and cultivate their work interest through day training programmes
  Refer/ network people with mental illness to local resources and services regarding their welfare needs
 

For service users who no longer wish to receive the service, caseworkers will establish an exit plan for them
